This form is for collecting personal data from employees with the purpose of completing background checks, either in-house or through a third-party checking / screening service such as DBS (Disclosure and Barring Service) or Disclosure checks.

There are 2 versions of this form; F-HR17A and F-HR17B which are the same except the B version includes additional medical questions.

Multiple versions of this form are also provided to allow further modification of the forms based on the assumption that you may need different versions of the form to provide to employees based on the job role and associated level of screening required.

Both forms include the following sections;

  • Personal details - various sections including; name, DoB, place of birth, identification details (passport / driving license), references / referees
  • Address Details - current and previous addresses
  • Employment History - details of previous employment
  • Qualifications - details of any academic and vocational qualifications or licences
  • Driving history - details of driving endorsements, collisions and driving health
  • Medical Information - details of medical conditions and (version B only) specific medical questions / declarations
  • Criminal convictions / Bankruptcy - details of convictions or bankruptcy
  • Declaration - declaration that details supplied are correct and consent for details to be held and transferred as required

This form includes a 'Record Retention Summary' section so details of protection and retention of the form once it is a completed record are detailed right on the form without any need to check personal data register for retention period.
